Requirements: :
- Fill the request form.
- Photocopy of Director / Leader / Owner's ID.
- Business License / Technical License.
- Photocopy of Establishment Certificate, Change Certificate which is ratified by the authorized institution.
- Photocopy of Balance Sheet (Neraca) (Special for CV).
- Photocopy of The Number of Tax Payer (for the companies which has a legal entity).
- Photocopy of Legal Certificate from Justice Minister.
- Photocopy of Disturbance License(HO) for all companies which have an environmental impact.
- Legacy Stamp Rp 6.000,-
- Photocopy of Technical License.
- Photocopy of Place Status of Business
