2. Photography sized 4 x 6 , 3 sheets
3. Recommendation Letter from Head of Agriculture, Farming and Marine
4. Employees Name List related to the meat and its bonding result attached with the Health Letter from authorized/Government Doctor
5. DFacility and Infrastructure List : Place of supply , store, equipments, meat and its bonding result Loading.
6. Statement Letter ready to obey the rules
7. Photocopy of the employee ID
