Requirements

• Transportation Principle License
  1. Fill the Request form of the Transportation License.
  2. Photocopy of the Agreement Letter of Route License (SPIT).
  3. Recommendation from Transportation Department.
  4. Photocopy of the Applicant's Identity.
  5. Unobjected Statement Letter from the neighbor.
  6. Statement Letter of having a garage (legacy stamp 6000).
  7. Information Letter of fleet number is being purposed.
• Transportation Business License
  1. Fill the Request form.
  2. Photocopy of the Approval Letter of Route License (SPIT).
  3. Recommendation from Transportation department.
  4. Photocopy of Statement Letter of the Route Handover from Past Owner (Legacy Stamp Rp.6000).
  5. Photocopy of People ID, Letter of Vehicle Number/STNK, Test Book.
  6. Photocopy of Disturbance License (HO) or Unobjectionable Statement Letter from Neighborhood.
  7. Statement Letter about he will not use the public street as a base (legacy stamp Rp 6000).
